Community, Hope, Opportunity, Independence, Careers, Empowerment, Success (CHOICES) is a program with services for children and adults with intellectual and developmental disabilities.
Family Support 360 (FS 360): helps people with intellectual and developmental disabilities and their families get the services they need to live in their own home and community as independently as possible. FS 360 is self-directed, which allows the person or their guardian to choose their staff and schedule based on their needs.
Assistive Daily Living Services (ADLS): The Assistive Daily Living Services (ADLS) program helps people with quadriplegia live independently in their homes and in their communities.
Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver: provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them.
