Medically Involved Children's Waiver (MICW): Provides employment path services, supported employment - individual employment support, waiver case management, discovery/career exploration services, environmental safety modifications, family training, individual directed goods and services, specialized medical supplies, supported employment - small group employment support, and vehicle modification services to individuals with physical disabilities ages 0-17 years who meet a nursing facility level of care.
Aged and Physically Disabled Waiver: Provides waiver case management, housing support services, and transition services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a nursing facility level of care.
Adults HCBS Waiver: Provides employment path services, supported employment - individual employment support, waiver case management, direct nursing, discovery/career exploration services, environmental safety modifications, family training - conferences and workshops, specialized medical supplies, supported employment - small group employment support, and vehicle modification services to individuals with intellectual disabilities or developmental disabilities ages 18 or older who meet an ICF/IID level of care.
Behavioral (ICF/IDD) Model Waiver: Provides employment path services, supported employment - individual employment support, waiver case management, discovery/career exploration services, environmental safety modifications, family training, individual directed goods and services, specialized medical supplies, supported employment - small group employment support, and vehicle modification services to individuals with intellectual disabilities or developmental disabilities ages 0-17 years who meet an ICF/IID level of care.
Children’s Extraordinary Needs Waiver: Provides attendant care provided by a legally responsible individual, employment path services, supported employment - individual employment support, waiver case management, discovery/career exploration services, environmental safety modifications, family training - conferences and workshops, specialized medical supplies, supported employment - small group employment supports, and vehicle modification services to individuals with physical disabilities ages 0-17, individuals who are medically fragile ages 0-17, and individuals with intellectual disabilities or developmental disabilities ages 0-17 years who meet a hospital, nursing facility, or an ICF/IID level of care.
