Persons with Physical Disabilities(PD): provides community-based, in home services to enable persons with severe physical disabilities to remain in their own homes and avoid placement in a long-term care facility. The provision of home and community-based services is based upon the identified needs of the recipient and available funding. ADSD assists recipients with accessing other available services, as needed.
Frail Elder Waiver (FE): provides community-based, in home services to enable the frail and elderly to remain in their own homes and avoid placement in a long-term care facility. The provision of HCBS is based upon the identified needs of the recipient and available funding.
Intellectual & Developmental Disabilities: Provides day habilitation, prevocational, residential support, supported employment, behavioral consultation/training and intervention, counseling, career planning, dental services, individual directed goods and services, non-medical transportation, nursing, nutrition counseling, and residential support management services to individuals with intellectual disabilities ages 0 or older who meet an ICF/IID level of care.
