Big Sky Waiver: designed to provide an individual with physical disabilities and/or 65 years of age or older, a choice of receiving long term care services in a community setting as an alternative to an institutional setting. BSW provides individuals with chronic disabilities of all ages a choice of long term care services that maximize their independence, provide quality care, and assure financial accountability. The individual must meet nursing home level of care

Developmental Disabilities (DD) Waiver: The DD Waiver supports choices and opportunities for people of any age with developmental disabilities to have meaningful lives in their community. Services are based on a person's assessed needs and priorities and may include skills training, community inclusion, residential and employment supports

Severe Disabling Mental Illness HCBS Waiver (SDMI Waiver): available to Medicaid-eligible members 18 years or older experiencing a severe and disabling mental illness need who require long-term supports at a level typically provided in a nursing facility.

Community First Choice (CFC) Plan Services: offers personalized, long term support for elderly and disabled Medicaid-eligible members allowing them to stay in their homes. Services, customized based on individual needs and living situations, encompass assistance with activities of daily living (bathing, dressing, hygiene, etc.) and instrumental activities of daily living (housekeeping, laundry, community integration)
