ILLINOIS
Children and Young Adults with Developmental Disabilities-Support Waiver: The Support Waiver for Children and Young Adults with Developmental Disabilities provides services to children and young adults with intellectual or developmental disabilities ages 3 through 21 who live at home with their families and are at risk of placement in an Intermediate Care Facility for Individuals with Intellectual Disability (ICF/ID).
Children and Young Adults with Developmental Disabilities-Residential Waiver: The Residential Waiver for Children and Young Adults with Developmental Disabilities provides services to children and young adults with intellectual or developmental disabilities ages 3 through 21 who are at risk of placement in an Intermediate Care Facility for Individuals with Intellectual Disability (ICF/ID).
People who are Medically Fragile, Technology Dependent: Individuals under the age of 21 who, because of the severity of their physical illness or disability would require the level of care appropriate to a hospital or skilled nursing facility without the support of the services provided under the waiver.
Persons with Disabilities: The Persons with Disabilities (PD) waiver provides services and individualized support to persons with a disability who live in the home. These services are provided to those who would otherwise require care in a nursing facility.
Persons with Brain Injury (BI): provides services and individualized support to persons with a brain injury who live in the home. These services are provided to those who would otherwise require care in a nursing facility.
Adults with Developmental Disabilities: waiver provides services and individualized supports to persons 18 years of age or older with a developmental disability who are at risk of placement in an Intermediate Care Facility for persons with Intellectual or Developmental Disabilities (ICF-I/DD).
Persons who are Elderly: waiver provides services and individualized support to persons who are elderly. These services are provided to those who would otherwise require care in a nursing facility. The Persons who are Elderly waiver is a component of the Community Care Program (CCP) provided through the Illinois Department on Aging (DoA)
Persons with HIV or AIDS: waiver provides services and individualized support to persons with Human Immune Deficiency (HIV) or Acquired Immune Deficiency Syndrome AIDS who live in the home. These services are provided to those who would otherwise require care in a nursing facility
Supportive Living ProgramPersons ages 22-64 years with a physical disability or persons aged 65 or over. NOTE: A Supportive Living Program (SLP) provider must designate which of these two populations it will serve.
